Registration for the 2014 Bushnell Reunion

Each shipmate attending the Bushnell Reunion
must print and completely fill out a Registration
form and mail it to Mike Wentzel along with a
check for all monies due. The check MUST be
payable to the "USS BUSHNELL REUNION".
Checks made out to anyone other that the
Bushnell Reunion as stated previously will be
returned. The registration information is used
for planning purposes, giving us the number of
attendees, dinner selections and trip selections.
If for some reason, you are unable to print a
copy of the registration form, E-Mail Mike
Wentzel and he will mail you a copy.
PLEASE NOTE: This is NOT your Hotel
Reservation. Each member is responsible for
making their own hotel reservations.
Registration Instructions

1. Click on the Registration form button. This
will open a PDF file.
If you are not able to open the PDF file, you may
have to install Adobe Reader. Click on the
button provided for Adobe, down-load and install
on your computer. Repeat step 1.
2. Print the form and fill it out completely. If you
are bringing more guests than provided for on
the form, print out another form and print Page 2
at the top. Double check your form for accuracy.
3. Fill out all meal requests, trip requests, and
number that will be attending the Hotel provided
reception on the first night. Enter your dollar
totals in the spaces provided.
4. Send the completed form and check for the
full amount to Mike Wentzel. His address is on
the bottom of the form. PLEASE remember to
make the check payable to the:
